The codex in the game for ps4 is broken with the new patch:( Won't keep some books or notes you read in codex to read later.

I don't think that's necessarily a post patch bug - I've noticed this playing on PC before the patch.
Not all books add to the codex though. Some start quests, it's just hard to tell sometimes with the way quests are tracked.
