Sorry if this is the wrong place to post this, this is as close as I could come (given the threads listed)

 

I seem to be encountering some weird endless dying loop glitch thingy.  I have uploaded a video for visual information.

Also, my game chooses to load from here, so now I have to lose progress

I ran into the same bug when I fell off a ledge in the quest "In Hushed Whispers" its annoying. They need to make the standoff of where the character respawns after going off-map a little further back. Please do this in a future patch, Bioware.

I encountered it in the same place as Etragorn. You can quit the game and start over with no damage done other than a few minutes lost, but it was definitely odd.
