I've finally killed all the attackers in the Shrine of Dumat, the next step is to find Maddox, I can see the quest marker and....there's no way out. Doors don't open, I can't find any hidden levers or passages. Also, no way to fast travel back to Skyhold. Is this bugged, or am I missing something?

Try going back to your last save at Skyhold and restarting the Before the Dawn Quest on the map. I had the same problem. Hope this helps if you haven't already resolved it by now.
