I recently adjusted tactics for some companions, setting a few abilities to preferred. Once I did that they stopped using any abilities. I just reset them all back to default settings but they still just seem to use default attack. 

 

Anyone have any suggestions?

Completely shut down the game. That always solves the problem for me.
