I sentenced Blackwall to join the Grey Wardens, and it wasn't until after that decision that I got the dialogue option to ask him about his feelings for Josephine.

When I talked to Josephine, she said he's been "anonymously" sending her flowers every week that he picks himself from a dangerous location. She also said she returns his feelings but because he will be joining the Wardens they have no future together.

Are they able to be together if you don't make him a Warden? I feel terrible now for ruining their happiness!

LOL, I'm pretty sure it won't trigger if you're romancing one of them! But yeah, leave Blackwall in prison if you want to be absolutely sure.
