Can missions disappear from the war table after a while? I remember seeing one mission where I had to stop Nevarra & Tevinter from going to war and could only use Josephine, but she was busy on another mission. And can't remember seeing anything stating there was a timer or such.

Also, I got Bull's Chargers and have gotten the mission to scout Haven, yet there is supposed to be a Scout Redcliffe (or Therinfal Redoubt, if you pick Mages, I picked Templars on this, my first playthrough) as well, which should open after certain quest is done, yet have not seen that one either. Can it really only be done after Champions of the Just, but is cancelled out after Haven is destroyed?

Depending on choices you make and story progression, yes, missions can disappear.

Well that is annoying as that was not mentioned anywhere. I assumed any mission would stay on the war table until completed (with any follow up missions happening if you picked the correct advisor etc.) and I thought they would remain throughout story as there are real-time timers, and Bioware seriously couldn't have thought people would wait with playing the game, to make sure they could get the war table missions done. Sometimes I wish some mechanics were perhaps spelled out a bit clearer.

Yeah, you'd think that but you'd be wrong. I have serious grievances when it comes to the war table. It needs an overhaul, complete with warnings, an improved interface, and a queuing system.

Bull's Chargers missions open up occasionally when you talk to Krem (usually after every story mission I think). You can get other missions by talking to other around Skyhold (such as your researcher Hellisma, I've gotten 2 missions from her). Story progress affects world events. So for instance I avoided going to the Templar/Mage camps in the Hinterlands until after doing the Mage/Templar missions (Champions of the Just/From the Ashes). So when I finally visited the camps there was no one there, because the story had changed. So operations should remain as long as you do not progress the story. No guarantees otherwise.

Does the dwarven slave one disappear after a while? I haven't done any main quest stuff since it first popped up but I had the advisors doing other things while I finished up some companion quests and now the mission is gone.

3 suddenly disappeared in Ferelden after I completed herbalist's mission too.

Bioware has built a dynamic system that responds to you actions. The world changes according to what quests you play & how far you are into the story. So things popping up/disappearing is part of the plan. This isn't Red Dead Redemption where you can complete everything. It's meant to be dynamic. Rejoice in it! It will make additional playthroughs more interesting.
