So, i've purchased up to tier 3 focus with inquisition perks. Yet despite the fact that there are 3 focus bars on my HUD, in combat my focus meter never goes past 1 bar. Does anyone know f I need to do something special to gain more focus in combat? Or is this a bug of some kind?

It's a bug. Try saving and reloading or traveling to a different zone from Skyhold. 

It appears to happen to characters who are revived in the middle of combat.

What Jester said. I find saving and reloading always works while zoning to another map doesn't always work.

If you die it goes back to the start of the tier.
