So, I transferred my first file over from the keep into Dragon Age: Inquisition. Their portraits and names appeared correctly the first time through. However, now that I tried transferring over two different saved files to DA:I none of the portraits and names are correct in the plalythrough world states. This includes the first game I transferred over.

 

What is going on? Everything else seems to be correct.

In other words the equivalent default versions of your heroes are showing up in the playthought states. That's suppose to happen because DA:I will change custom heroes into defaults since it can't read the custom name or portrait.
