These all are expensive. Not every developer can afford them.

Dear OP, I think that game will appeal you more than DA serie (wich I don't want to become too realistic) :
They focus so much of being realistic that they didn't implement 3rd person view and even the hardcore fans who want the game be realistic as much as possible rejected that option. And there is no fantasy in it since it's realistic you know...
That looks really good. Very much what I look for in a game. Probably a mount and blade: warband++

Dragon Age Inquisition is "gritty" in some aspects but with the demons, undead and visuals, it's mostly dark fantasy there. In other places, it's like the game was too scared to convey certain themes, the game wouldn't show decapitations of people you executed, there are no visible brothels in the game or criminal organizations taking advantage of the chaos. It would have added to the theme if we came across bandits pillaging a village for example. We hear of it, we see bandits in the area but they're not doing anything.
Concerning brothels, part of having a "gritty" atmosphere would be to show this type of life in the city even if the player can't use the services themselves as they could in Origins and DA2. Mature themes aren't tackled at all like they were in Origins (i.e adultery, rape etc) and none of your companions betray or walk out on you.
And where are all the blood mages? What happened to desire demons? I don't recall seeing any dark rituals in this game or demons possessing children. Those concepts really amplified the "dark fantasy" theme of Origins.
Some parts were done well though. I recall coming across an abandoned camp in the Exalted Plains with burnt corpses. One was holding a dead child. Also, all the destroyed villages and towns we come across, the war torn plains we explore and the clashing factions do convey some darkness.
In fact one theme to convey something more realistic and gritty would be just to give us a sense that people on our side could die like with Mass Effect 2. Inquisition doesn't have that and some content is just terrible fan service (the stupid god child, Hawke and Morrigan).
In conclusion, while I'll say that Inquisition still retains the dark fantasy of the series, it's still not as "gritty" as Origins which I guess is what you're asking for rather than all the dark fantasy. Origins wasn't afraid to kill a companion off, have them betray you or allow you to kill them. Origins dealt with rape (i.e Leliana's song, City Elf origin) and The Warden was able to be a ruthless murderer to just get the job done. Oh and you could use people for sex just like in real life.
The other things you're asking for are just realism in terms of the world that Bioware has never done. I too would like to see actual NPC schedules for once and a living breathing city.
Oh that looks interesting was read the info on their kick starter.
That whole mystery Eastern European investor with tons of money part sounded a bit dodgy; game looks fantastic though.
Kingdom Come was funded 11 months ago and since then, the team have been regularly divulging everyone with the latest developments, uploading developer diaries and they even released an alpha for backers to test at one point. So don't think it's a scam as it has long since been funded and kickstarter teams that are scamming quickly disappear after reaching their funding. Warhorse has been around for 11 months still after...

Fallout 2 had permanent deaths, 1 shot kills from weapons that could easily cause these perma-deaths, the option to sell your spouse into slavery or pimp them, use of condoms, alot of evil quest decision paths, quid-pro-quo with a female NPC in exchange for rescuing her husband, ability to say "f" it and kill everyone in town because you can, and even child killing and the animation gore against their bodies to come with it - things developers could get away with before ratings got really strict.

Can't see how this will work out. Either Solas or Varric will babysit your character by bullying low level enemies or your character gets mercilessly killed by powerful enemies initially.

Characters do more than just stand around
When I rest at camp, after the fade, I would love to see the characters doing something, sitting, cooking, etc.
This. NPCs are like statues. This is not a living world!. They have no schedules.. you can't talk to them. It feels scripted when the first time you approach them. Heck it is SCRIPTED!. This is one aspect of the disappointment i have together with the limited skills and action-ey combat. Otherwise it's GOTY for me. But not quite.

Yeah, I might even make a new hardcore character in Diablo 3, for that is punishing me as a player in multiple ways.

I played that on classic Iron man and loved it.
That is another good example, A fantasy RPG like X-com. I wish the war table was more like the x-com base too. The treat of the enemy continued to grow and you had to constantly keep on top of it or lose all your support.

I played fallout 2 so much, it was the game that made me switch from using the mouse left handed to right handed from that point forward. I was playing it so much that every click of the mouse shot pain up my arm through my wrist. I had to switch hands to keep playing.

you can equip even toerings, every cm of your body can be either customized or rolled and experience is only given if you success in your current work feature, plus the horrors you mencioned in Harnmaster and also adds a detailed Crime and punishment system for each race.
you want the most realist RPG, this is... sadly when we play making it too realist will end in being a BAD GAME. Not even the stupid OP shooters are realist, everyone takes a dozen of bullets before falling and has wolverine regeneration, the only game that i saw with REAL BULLETS was manhunt on the nintendo Wii, 3 shoots you are dead, no regeneration and if you get a head shot you die instantly (unlike stupid FPS games where you can handle up to 2 headshots), everygame needs breaks from reality otherwise is unplayable and stressfull instead of enjoyable and successfull, nuff said

Different Experience Levels
This would unbalance the game, and force players into taking certain champions because they are stronger. Not to mention, the main character catching up to them over time ruins the idea anyway; Whats the point of making someone more experienced, then giving them the exact same amount of experience as a level 1... and ending up equally experienced? It doesn't make sense, and it gimps players who want to play their own way. It wouldn't work for a DA game, not for the companions.
Character perma-death
Yes, this is great when its doen well. I was disappointed there was no death for the companions. Characters in previous games had crisis points where they could lose their life... DAI
Realistic time...
Sounds sort of fine... But in practice... No. Can you imagine how boring it would be to see players actually assemble armor? What sad little mini-games developers would make to attempt to make it more interactive? Actually taking time to craft things would not be fun. People already went out for the materials; That took time. Forcing them to wait more time for immersions sake should be a toggle, at most. I do love toggles.
Realistic Dialogue
They already have like a billion lines. Adding a line for every little thing would take so much time... But It would be awesome. Its the little things that make a good game great. The question is, is it worth all the effort? Could the effort be better spent elsewhere? Probably. They do have a deadline and a budget, you know.
Idle Characters
I was sad to see this wasn't implemented. It makes characters feel very fake when all they do is stand in one place, not speaking to anyone, not doing anything, not reacting to anything around them. Imagine how lifeless Skyrim would feel if the already-robotic NPCs didn't have the schedules. Cassandra trains 24/7, so its not like they missed everything. But Iron Bull should be drinking and partying, Vivienne should be in her room with visiting nobles. And most importantly, don't get them to do the same thing in the same place. Have them move around, sure it'd make it slightly more difficult to find them, but it'd make the more real. A minor inconvenience, at worst.

But id preffer RIFTS.
I've been waiting a llooonnngggg time for someone to make RIFTS into a video game. When I first heard about the MMO Rifts I was so excited I was finally going to get to play as a Juicer or Glitter Boy. As you might guess, my dreams were shattered when I finally saw the game and realized they just stole the name/concept and that it wasn't even based off the pen and paper game.

This is exactly why Gandalf was absent for most of the story.
instead of saying you want a developer to change their games to meet your needs, why don't you go look for a game developer that makes a game you would enjoy?
Go tell that to all the people who get mad at those of us who wanted the DA series to stay closer to Origins.
As to the comments about the Witcher, we all know the sex cards are stupid. However, the game was "mature" in the sense of the setting and the issues going on in the world. There were several examples of situations which were either morally gray or the side that was wrong at least had a legitimate grievance. DA2 tried to do this with Templars vs Mages but blew in the end. On the other hand, there are some completely bad guys that you will simply want to take out because they do bad things. It is a good example of Dark Fantasy.
