Electrical Staff: Tesla's Anachronism
Her armor: Embrace of the Elvhenan
Cassandra's Ax: Cassandra's Fury
Cassandra's Dragon Shield: Pride of the Pentaghasts
Blackwall's Ax: Warden's Redemption
Blackwall's Shield: Against the Wall
Blackwall's Armor: Burden of the Grey
I have a few others. I'll check the next time I have that playthrough loaded up.
Heh, i named Blackwall's shield "The Black Wall" and his mace "Rainier's Redemption".

Made a bow with Bear Fur. Called it "Bear With Me".
Made a bow with Dragonbone. Called it "Dragon You Down".
Made a bow with Phoenix Scales. Called it "Scale It Back".
Just stupid puns like those.
With my DW, I named the daggers Rose and Thorn. It would change depending on materials. So Ever Rose & Storm Thorn, for example.

Also a bit because I know myself. I can easily spend an hour looking for a name for my PC that's just right, with the right meaning. (I have a set cast of names now though that I recycle between RPGs, so that time sink is mostly gone

I just finished reading the dark elf trilogy and I made my character look like Drizzt in the sense that he has white hair and purple eyes. Therefore, I name every armor I craft for him "Piwafwi" ![]()

I play as a Dalish rogue Inquisitor, and I gave her a tier 3 spiked longbow that I named "The Thorn" after one of my favorite Wordsworth poems:

Wordsworth often wrote about the dispossessed and the downtrodden, so I thought there would be an appropriate thematic resonance with a Dalish Inquisitor. I also crafted her a backup bow and called it "The Queen of Limbs," after "The King of Limbs"--a Radiohead album / thousand-year old oaktree in Wiltshire:

My Rogue uses two forward bending daggers. In real life they are called a Falcata, except they never had a second blade after the handle to my knowledge. As a bit of nostalgia I named mine catabolg (left) and catabolg (right) after King Arthur's sword in its original name. Ex-calat-bolg became Ex-calibur: Excalibur. So, a bit of Celtic history playing through as my character is an elf. When I'm feeling inspired I'll name other things in Manx Irish as it is the most fun and exotic of the surviving Gaelic languages.
After getting extremely frustrated from 70+ failures to craft a sword using dragon materials I lost that romantic sense and named everything else I made Sera's this or that. Solas' this or that, etc. It worked out a little better anyway as I stopped accidentally selling everyone's stuff or de-equipping it accidentally.

I'm probably gonna come off as a massive nerd, but whatever...
My HM DD Rogue:
RH Dual-curved dagger: Havoc, Blade of Treachery
LH Dual-curved dagger: Midnight, Blade of Darkness
Armor: Nightfall Coat, Nightfall Hood
(Havoc is the mythical weapon for Brigands in EQ2, Nightfall is the assassin raid set in an old expansion)
My HF Archer Rogue:
Bow: Eaglesong
Armor: Windrunner Coat, Windrunner Hood
(both are names from DOTA)

I forged some daggers and named them Glass Dagger. Had to get the Skyrim theme going... (first game I played where I get to name my weapon)
I always name helmets "xxx light-binding device". You get the point.

On my first playthrough, I went with an elven gods theme for my Dalish Inquisitor's weapons. The first bow she made was named "Fen'Harel", which was... an interesting coincidence. Her endgame bow was named "Andruil".
As for armor, I just stuck "Lavellan" in front of the standard armor name. So, "Lavellan Sturdy Coat", "Lavellan Scout Arms", etc.

I've named Vivienne's staff "The Van Winkle". It deals Ice damage, obviously.
It makes enemies stop, collaborate and listen. Turn off the lights and it glows.
