Confused with crafting

Really, I like the crafting system. Nevertheless, I don't know exactly, why the results are different:

 

I craft T1 and T2 armour. It always comes with empty slots for arms and legs. Good, because two more options to buff the wearer.

 

Now I tried a T3 piece, but it comes without these slots. I thought it might be one of these random luck things and reloaded several times, but the result was always the same: No slots at all.

 

My last attempt was, to put in a fade touched thing which grants a 100% success chance for a master-work. The result was the same, just without the buff of the previous item in the top slot.

 

My questions:

 

1) What must I do to craft armour with two slots?

2) What exactly happens with so called master-work items? Is it worth sacrificing the top slot for such a x%-chance item?

Some armors and weapons don't have upgrade slots.

There are armors that have set appearances which you can't alter with arms and legs slots.  The Legion of the Dead, Grey Warden, Light/Medium/Heavy Armor of the Dragon, Dalish Scout/Keeper/Warrior Armor, etc. are examples of this.  The Defender, Battlemaster, Enchanter, Apprentice, Hunter, Prowler, etc. armors are those that you can add arms and legs to.

 

What's the tier 3 armor design you're trying to craft?
