So I'm at the Well of Sorrows with Samson and I have to fight him but Sera and my rogue Inquisitor keep dying. I keep going after Samson first but no matter what, I keep dying over and over again. Any tips? Should I focus on the minions first instead?

take out the adds first, then focus Samson. If you have some Jar of Bees they work wonders, same with the Pull of the Fade (I think the name is right) that the Inquisitor has, it is their focus ability that does AoE damage.
Try to keep Sera if she is on a bow at max range. this will reduce the damage she takes and the chances of her getting stunned.
Once the adds are dead the fight is similar to Meredith on DA2, except there are no more adds spawned.
If you have the Regen. Health Potion on your characters use them esp if you max'ed out the upgrades if both of your melee use them then you will regenerate about 25% more health than the damage he is able to put out. The potions AoE field is a great bonus.
