This time it was all on purpose
You just have to use Hissera, the Saarebas in Multiplayer, go to Fereldan Castle, target the Dragon and use the third stance of Asaara's Leap to jump to the Dragon.
Most times, it will just teleport you back to the place where you started off, because you're out of bounds, but if the Dragon flies over the castle you can even end up in the next Zone
Only do it together with a friend, so they can get you back down, if you're stuck, in opening the door to the next Zone.

I was playing through Bull's personal quest again and Gatt died during the last fight against the Venatori mage. So he just disappeared right before the last cutscene and I got some really weird camera angles while Inquis and Bull were talking to air. Thanks Gatt!
