I never ran into the problem until this week. Played four games last night: Wiped the first, finished the second, and the last two were key-drop bugged. I think one was in the elven ruins, the other in Orlais.
I don't recall if there were critters in the elven ruins, but there was a red dot on the mini map for a solitary left over enemy which was not on any accessible portion of the map. Never laid eyes on it, just saw it on the mini map, like it had spawned badly, or fallen off the playable area into a nook or cranny.
There was an unusual rush of critters in Orlais just before the door to leave the zone, and the key never dropped. There was no orphaned enemy on the mini map that time. After two bugged games in a row, I gave up for the night.
I'm on PC.
While I don't need a blow by blow status of bugs being worked on, a simple "we're working on that one," would help a bit. I doubt to ever hear it, but it would be nice.
