I went to create a male elf character, trying to make him look handsome (had no problems with the Qunari, Dwarf, or Human).
All I managed to do was make him look effeminate.

What's the difference in appearance between a "forced" starvation victim and an "unforced" starvation victim?

Oh, that has an easy answer.
"Unforced" starvation victim don't get their arms beaten to the point they fracture and get inward. I imagine that happened when the inquisitor, at some point before the start of the game, dared to complain for the lack of food.
Although my theory is that the elf male inquisitor body type is just a horrendous mutation from the legendary beauty of the descendants of an immortal race, with a millennial legacy, that Bioware told us all the way about this race.
I'm sure they will figure out a way to explain us how the inquisitor's body morphed little by little into a Mantis religiosa type in a future DLC.

The weird thing is is that the male elf's model changes in the romance scene with Cassandra (not sure about other romances.). My character went from stick skinny to a wiry guy with decent muscles. I'm on PS4 so I can't get a pic...hopefully someone else can provide one.
