This has probably been answered before .. and I am probably missing the point of it, but does the Tapestry in DA Keep automatically alter and change as my game progresses through DA:O and DA2?

No it does not is my understanding, there is a Save Game Reader that is supposed to extract the choices for you but I've just been taking notes whilst playing.

Really?.. That's weird. I've only just started my newest DA:O save and it's at least recognised the name of my character and race/class etc?

The Keep can read your "characters", specifically, but not any choices. Similarly, your name, appearance, and so on are pretty much moot -- the Keep doesn't really register them, beyond the Your Heroes section thing.

You'll have to register those choices in the Tapestry yourself.
Fortunately, since you're playing now, you don't have to try and think back to what you did X years ago. (The Journal helped me some with this, but other stuff I did have to guess on. Oh well, I remembered all the important stuff.)
