I've noticed that sometimes a despair demon does its dodge thing straight through locked doors, getting itself stuck there and leaving you unable to kill it unless it floats back through.

Animals seem to have this issue a lot, I've seem wolves poking their heads through the door and recently I saw a phoenix that would stick its head through the door if you came close, then wander off if you backed up. It was visible on the map when close, but after it backed up a little it was no longer visible.
Seems like the key not dropping issue is just enemies getting stuck behind doors/spawning in the wrong place :/
