I accidentally ran over a rabbit in DAI with my horse and it made a horrible squealing sound as it died. 
It's worse because I have a pet rabbit.

I killed all those nugs in the Crestwood caverns inside like 2 seconds and got instant massive disapproval from everybody in my party, except Vivienne.
Everyone was like "But-but the nugs.... 
" so I had to reload.

Sick- Boned the Desire daemon instead of rescuing Connor. That was for my elvhen Circle mage. My City Elf slaughtered all of those shems at the estate while rescuing my cousin and enjoyed it.
Filthy- Broke up the elvhen couple in a certain fashion.
Repulsive- Depending on perspective, someone may find it repulsive that I had the OGB with Morrigan. If not that, then perhaps those surprise options at the Pearl are a suitable replacement. Then again, there's the fact that my noble Dwarf is a deadbeat dad.
As can be seen, there is a running theme in those choices. My favorite character was a rogue City Elf who thought the Dalish were kind of obnoxious and hated those damn shems because... reasons.
DA 2: Can't really say I did in this. Have to play through it again and try some choices out.

Sick: killed the Circle Mages.
Filthy: wiped out the elvhen clan.
Repulsive: Broke up Aveline with her boo.

Sick: Killed all of those wonderful High Dragons. Now all of those dragonlings have to scurry about without a mom.
Filthy: Gave Viv the fake heart.
Repulsive: killed the lady right in front of everyone at the Orlesian Ball.

I did something pretty repulsive today...I put Viv in an atrocious outfit with an ugly hat and then forced us to walk around Val Royeaux. Dammit Viv, I love you but stop moving my furniture. :'c

On a meta-level, it is a bit skeevy, but the Inquisitor not being privy to all the events of TME does make things easier. Heck my Warden helped Bhelen assume the throne, and she knew he was a total dirty scoundrel XD
