I would like to go back to the Hissing Wastes. There is a Dragon I left behind to kill at a later date when I was a bit more stronger, but now I can't get back there. I can have one of my advisor's organise gathering resources, but that's it. I want that Dragon dead! Am I now locked out of it forever?

You're not locked out. I killed several dragons after the end of the game, but I also experienced not being able to travel by war table.
Pull up your map (on PC you can just use ESC), choose "world map", pan left, voila!
Not using the war table may have an effect on the banter bug, but I haven't lost banter in 160+ hours traveling this way.

I can't get to a fade rift to close it, the door to the cave where you fight to get to a boat (Storm Coast) is shut AND there's no "X" to open it either.
There goes that cheevo

I shared your frustration for the longest of time...
Turns out, you have to complete some of the main story before that cave unlocks.
Then there is an operation to scout that area (from the war table) and bam, it unlocks.
