I just joined a game where the whole party did not lose any HP the entire game. Is that a bug or is that a hack?

That's a hack. Please issue a hack report from the ingame when you see it.-

One more question: How do i issue a report ingame?

It is a trainer, ive seen them used as well.  guy was 1 shotting everything and not losing HP, no cooldowns either.  was crazy.

To report a player on Origin:

1. Open your Friends List
2. Click the '+Add a Friend' button
3. Search for the Origin ID of the user you want to report
4. Click on their name to open the user's Profile
5. Click 'Report User' and tell us what sort of problem you're having

Otherwise,  it is recommended that you submit any or all of the following using the steps below:

1. The screenname, Gamertag, PSN ID or Origin ID of the offender.
2. A screenshot of the action as it is taking place.
3. A screenshot of the main score screen.

Note: Having a screenshot is not necessary, but will help with taking action against the offending player. You may also submit a recorded video of the game if uploaded to YouTube.
