Even in the middle of a cutscene it would be nice to see save points right before you have to make a major decision, wardens exiled, halamshiral, well of sorrows, etc. Instead of having to run around and do everything before the decision all over again :-/

I suspect they do that on purpose to have the decision have more impact and you're less likely to reload.

It just makes it more annoying, not less likely.

Meh, second time playing through the game I know how one decision will impact the world and I accidentally chose the same option the 2nd time. I think it would just add to the replayability

Hard to say if it is a design issue with so many open issues. DA3 has some odd save timing at times. If the game made a profile save it will load the decisions regardless, but previous milestone save might.
