Tranquility is awful, but I think there are much worse fates considering it is potentially reversible. Sure, it may not happen often..but there is at least a sliver of hope.
On that note I think darkspawn corruption (outside of the Joining..which is different) is far worse. You'll be driven mad and will go down into the Deep Roads, where eventually you'll become either a slave or cannon fodder for the darkspawn. And the fate of women who are turned into broodmothers...even worse.

I would say that lyrium addition is the worst of that particular selection. With tranquility you don't mind that you're tranquil, that is the whole point. Sure, right now you may think that the result of tranquility is horrific, but you don't feel it when it's done.
With demonic possession, the darkspawn taint, and red lyrium corruption you basically lose your mind, so you aren't entirely aware of what you were or what you're becoming, you basically live in the moment at any given time.
I tend to feel that lyrium addiction, or any sort of major drug addiction, is the worst because when you aren't high there is constant anxiety over your next hit: when will it be, will I be able to take it (find some, afford it, etc), fear of not having access, fear of increased resistance, and on and on. Then there is the horrible state of existence if you begin to go through withdrawal.
The only withdrawal I've experienced in my own life has been from lack of caffeine and it wasn't pleasant. I can't begin to imagine the horrendous pangs for much stronger substances.

Tranquility for me, no question. As an artist and very empathetic person, I would be losing everything that makes life worth living to me. In fact, you don't even need to be an artist or an emotional person... losing your hopes, your dreams, your desires? What's the point after that? To be some husk of a servant to someone, pretty much. I was offended even using tranquil as resources at Haven/Skyhold, and when Cass told me it was reversible I wanted an option to ask how so I could help them. I wouldn't even punish the most reprehensible person in the world with tranquility.

Yeah, one thing about the DAverse is that optimism is not something you get a whole lot of, ever.

Broodmother, without a doubt. The process of becoming one is horrifying and then you have to live with yourself as you feel the corruption worming it's way through you. At the end of it, you've lost all your humanity (dwarfmanity? elfmanity?) and are just a mindless lump of flesh, endlessly giving birth to darkspawn until some brave Warden or Legion of the Dead gets far enough into the Deep Roads and puts you out of your misery.
A runner up would be possession, because it seems like you retain some sense of your consciousness. It would be terrible to watch a demon drive your body like a go-kart, wreaking havoc with your power.
