So this is my first post on this message board. I read a lot but usually don't post, however DA:I has me geeking out with the end.
So here's my personal theory on some of the things:
The maker was a spirit of imagination and he/she/it imagined a world in the fade so it became real there.
Other spirits, whom the game has shown have a desire to belong, were drawn to this new world.
Those spirits imagined themselves to be part of the world and thus became the first elves. Similar to how Cole became "real".
The elves over time forgot their own origins, and the maker seeing his/her/its creation overrun by other spirits and cut it off from the fade, thus creating a separate world.
Some of the elves bound themselves to spirits. Perhaps to gain more understanding of where they came from or for personal power. These spirit/elf hybrids were the elven gods.
I also think that the old gods were spirits bound with dragons thus more intelligent than normal dragons.
I think that the ancient imperium used lyrium along with elven artifacts to enter the fade. The magic itself corrupted the lyrium thus creating the blight. So it started in the lyrium and those magisters who used it and spread from there.
Anyhow, those are some of the things I think after playing DA:I, I look forward to the official explanation, but until then it's a lot of fun to hypothesize.
