Hi,
is Companion (dis)approval necessary to unlock their content (i.e. quests and romances and possibly other things) or is it just for flavour and tone?

I couldn't unlock Dorian or Bull's quest til I got their approval to a certain level. Same with Solas.

Yes, approval is meant for unlocking things related to companions. So quests and romances. I know that high disapproval will not necessarily make them leave (that might even be impossible unless the story requires it).
An example is Vivienne: I read a couple of threads asking how you could get rid of her once you recruited her, and people were unable to, no matter how much they disagreed or how much disapproval she gave, she never left.
It may also determine certain final events, like in Dragon Age 2 where approval determined who helped you and who opposed you in the last segment of the game. I can't confirm that for this game as I am not done yet.
