I've read that completing this operation gains both an amulet of power and approval from Cassandra. I'm on my third play through of the the game and have yet to see this operation anywhere. Can anyone tell me if they have received this war table op and how? I'm starting to think it's bugged and I haven't been able to find any kind of online resource to tell me if there's a prerequisite to gain access.

I've had that operation, though not on every playthrough, so there must be a prerequisite.
