More than that, you can get a pretty good sense of where you should be by just considering how much approval/disapproval you've seen from them.

I'm heading through a play through with the intention of romancing Cassandra in the story, but I've also just recruited the Mages as allies. I've lost count of the number of slight approval gains, a few approval gains, a few slight losses and one 'greatly disapproves'... And now I have zero idea where I'm at with her approval. Is there any way to assess where I'm at?
You can tell based on her verbal feedback. What does she say when you want to ask her questions? Are you able to ask about her brother?

there was a way patched in....but its the only way.

I have come to the concluson that the approval system is a red herring. I started my first playthrough and eventually got to the point that I didn't like the things I was saying to court approval. I started over and began to ignore the approval/disapproval of any companion. It had zero effect on my ability to romance any companion that I could discern.

I think Bioware may have employed the a similar approval/disapproval system to the one they used in SWTOR. In that game, if an approval event typically gave you 5 points of affection, then a disapproval event would give you -1 point of affection. That way, just by playing normally (i.e. not intentionally trying to alienate a companion) you were bound to build net affection.
It would explain why they hid the approval numbers from us. It would also explain why, despite disapproval messages, it is hard to totally alienate a companion.

Approval hardly matters in this game. No one will leave you and most companion quest don't even require approval to unlock.

That is not true. Lying is bad, and you should feel bad.
(if you don't believe me, go out of your way to make them dislike you and see what happens. And try to complete the game without receiving too much approval to see if their quests unlock)

Yeah, but she says she'd rather discuss it at another time. I'm just about to start the In Your Heart Shall Burn quest, so not *that* far in. This is the only serious disapproval she's offered so far (and to be honest, conscripting a group of Mages after what I'd just seen in the previous quest would be waaaay too petty), so I doubt it's that big a deal, but I'd still like to know.

I had her tell me about her brother at Haven before In Your Heart Shall Burn. Then again, I had taken all but one flirting option with her at that point. I don't remember if this was before or right after allying with the mages, though.

Not true. In my 2nd game I gained a ton of disapproval from Blackwall & he left my party for good & his quest was no longer available. It can happen with other companions, too.

I think the question was more about how hard is it to lose a squadmate due to disapproval. Blackwall in particular seems to be easy to rack up approval with so I doubt you could could get into this situation by accident.

Characters will stop providing approval once you've maxed out at 125 (I think that's the max, but the numbers are hidden from view). However, if you get disproval at this point, then you can gain more again.

I like the current system. Unless you're an outright ******* to them, it's pretty hard for you to lose them as companions. They just won't open up to you unless they like you, like how it is in real life. I can totally see how an aggressive or extreme-type player can drive some off, especially if you ignore their requests while pissing them off, just like in real life!
It wasn't hard for me to befriend them all as I self-inserted as a friendly guy.
You notice their attitude towards you changes as they gain approval. Unlocking cutscenes is also a good metric for how you stand with them.
Just keep gaining their approval and you'll unlock interactions. When you stop gaining approval you've capped and therefore have most likely exhausted all cutscenes until the next plot mission is completed.

You'd have to be damned friendly to get away with a self-insertion while befriending folks.
