I believe its a mission early on that sends you to Val Royeaux and after talking/finishing a quest there, there was an arrow that someone shot at me that had the quest, think thats it, all happened when I went there for the 1st mission to Val Royeaux (unlocked on war table) " The Threat Remains "

Yup when you go to Val Royeaux to talk to the chantry sisters after the cut scene with the templars someone will shoot an arrow near you with a note attached. You have to find 3 hidden red things and then travel to a hidden courtyard via the world map. This is where you will be introduced to Sera.
It is early in the game.
This is also where you will meet the messenger that invites you to Vivenne's Salon to recruit her as well.
