I am running a Paragade Shep and on Virmire did all I needed to do to take the heat off Kirrahe's Team, gave him Ash, completed all four objectives, chose Ash to save, etc...but Kirrahe died ... is the first time I have lost him ... anyone know what else needs to be covered to ensure his survival?

Destroy Triangulation, Communication and the Flyers. Dont divert security to him and his crew. If you do those four things, he lives. If he dies, might have been a glitch. Check the Mass Effect Wikia page. It might answer it better than I can... though I have replayed the Mass 1 storyline at least 15 times........

Yep, destroyed them all and didn't divert...i have played it many times over also, but this is the first time I have ever lost Kirrahe ... the only two things I did differently was Liara hadn't been picked up yet and gave Ash a renegade response when choosing who lives, chose her but she told Shep Kirrahe was dead...checked where he normally is located on the Normandy, he wasn't there ...Yes, I will check wiki, forgot about that, thank you
