I'm required to defeat three Champions in the exalted plains. One is located near the Riverwatch Camp along the desolate bank. The second near Fens Camp in the grove heading towards The Dead Hand dungeon, This area being unlocked via War table operation. The last in the Shrine to Sylaise located south following the river from the Dalish camp.
I've defeated two champions but wasn't really paying much attention to what order. I believe I've defeated the desolate bank and Grove champions because I can no longer find them. But the third champion located inside the shrine, doesn't register after I've killed him. I'm only assuming this is the last required champion because He's the only one I can seem to find. Like the other two have de-spawned.
Been working on this quest for over an hour, any explanations or solutions welcome.

Its official this quest is bugged for me, I've deafted the champion in the shrine 10 times with no avail. Plus the other two refuse to spawn, along with my hopes of being a champion. 55hrs in Nightmare didn't prepare me for the let down.

UPDATE.
Ok after killing the Champion inside the Shrine to Sylaise for the x amount of times, He finally drop the Heraldry. So if your having the same problem as I was. Rinse and repeat the process took me 10-20 kills before it worked, which to say the least is very frustrating.
Locations
Riverwatch Camp - Along the Desolate Bank River heading toward the broken bridge.
Dalish Camp - South along the Halin'sulahn River up past the Wolf Statue. Inside the Shrine to Sylaise.
Fens Camp - Ghilan'nain's Grove through the first stone archway. Continue until you see two additional archways, take the right hand side.
Side Note - This Area is unlocked via the War Table Operations.
NPC's Are Marked with distinct Red Feathers worn on their heads and carry large shields.
The book is located by Blackwall in Sky Hold and the construct a standard is a requisition request from tables like the ones found in camps usual outside near the tents.

I've just added this image to the Dragon Age Wiki with the exact locations of the champions. This corrected the image that was previously there which had the wrong location for The Desolate Bank champion. Hope it's helpful. (Click for 1080p image)
