I have Cassandra, Iron Bull, Vivienne and A Mage inquisitor
Cass is specced into sword/shield and vanguard
Iron bull into two handed and vanguard
Vivienne is specced into fire and recovery/support
Quizzy is specced for full ice and reviver/support.
I have grinded the hinterlands, wounded coast and fallow mire to level 11
All are wearing crafted armor and have crafted weapons.
I really need some strategy advice here since everything I have tried fails.
I have plenty of saves to respec and rebuild so any suggestions would be helpful thanks.

I also found it quite impossible to defeat mid boss, so I just let others die and manned the trebuchet.

Is your tank dying or are you other members dying? As long as you can keep the tank alive this battle will eventually be won. just use your other members to whittle down the the archers then the knights/guardsman and finally denam for last. You may need to kite around a bit and maybe even manually control the tank.
Really the only thing that matters is making sure your tank has turn the blade, turn the bolt, shield wall and enough hp/armor to tank long enough to win.

You can do as they have suggest above or just keep turning the crank only a few turns at a time, you'll get lots of little groups of 2 mobs where you shouldn't take damage and every so often a big wave. If you clear them all before continuing you won't be as overwhelmed. The 3rd big wave is the boss, clear the trash and you should be able to kite Denam solo on your mage just via winters grasp and fade step.
If people die against him, run out of range and res them. Fire is useless against Denam isn't it, 1 damage so Viv would be doing squat in that fight, also pick up shield bash if you haven't on Cass, just to help shatter that guard when he brings it up. He is very vulnerable to chain freezes, with 2 mages using winters grasp and ice mine you could keep him frozen permamently if you don't shatter.

Iron bull has a master crafted greats word with +23 constitution and Cassandra has seekers shield.
Problem is the enemies go straight for the mages and they can't lose the aggro. Meanwhile the Warriors can only hold for so long and they cant hold all the aggro.
Kiting seems to fail cause of how quick the boss is.

If your mages are grabbing initial aggro, try going into tac cam and double clicking move on the left hand side of the crank. A double click will tell them to hold that position, and being further away from all the spawns on the right means they won't get intial aggro from any mobs, leaving them generally to wait on the melee.

I don't know if it's the case on nightmare, but on hard the behemoth guy is vulnerable to sleep. So if you have any rogue with a bow and upgraded Full Draw they can pretty much keep the boss out of combat the entire time, or as long as it takes to get the crank thing done (and for the kiting person to gain some distance). You could also freeze that guy which, while lasting less time, also puts chilled effect on them which should slow them down.Kiting seems to fail cause of how quick the boss is.
edit: for kiting, there's also mage skills like the fade step -- it has short cooldown and moves you a large distance.

I had no problems with him. Cassandra caught his friends, and he just stepped on the ice mine and mages killed him... It was quick, with ice staves and energy barrage.

Please note, I'm a wimp and was playing on normal not Nightmare (so my experiences may differ)
First play through with a mage, I had this issue. While playing 2nd time with my dual wielding rogue, I finished the scene and went 'Where's the boss'. I think I actually just mowed him down so fast I didn't really notice him much...not sure. However, a rogue deals tons more damage than most mages (to single target)...so the second play through was much much easier.
I was really nervous with the dragons on the rogue though...but after I crafted some stuff with the tier 3 mats (knives for me and greatsword for iron bull), we mowed dragons down. Then I reforged a dragon bone greatsword for IB and a tier 3 dragon bone bow for Sera, and dragons fell like flies.
It was quite a different experience than with my wimpy oft dying mage. Not that rogue didn't get too much agro and die sometimes too..but stealth was his friend.

One of the most common problem with companions ignoring previous orders is because they have their behavior set to follow controlled character. Because everytime you switch characters, their behavior changes.
I didnt try to crank the wheel while letting the companions deal with the enemy, basically I turn the wheel a little and immediately deal with enemies that spawn.
When I was playing a Mage Inquisitor, I took direct control of my 2H warrior because the AI can play mage decent but sux at playing 2H warrior. Earthshaker+ and Charging Bull both very effective.
