If so, to what scale?
Will choosing necromancer or reaver cause people to fear/frown upon me? ETC

As all other Dragon Age games, the best you'll get are one or two comments from people.
I don't play as a mage, but I've gotten the impression at multiple points that if you do, the comments from people would likely be different. Perhaps more emotion or detail.
It changes if you are a mage/non-mage. You have different dialogue and choices.
Your spec is less impactful but it's mentioned a few times, some more than others.
Had a conversation with Iron Bull about why I chose to be a Champion. I got to say "I really like being completely unkillable" and he laughed.
Small little dialogues about it that are unimportant but still fun.
I also got to have a conversation with a major story character in a main quest about my specialization.
It doesn't change the plot, you do get comments and dialogues on it.
Templar is pretty much the only spec that gives you unique dialogue and that's only when talking with Cullen.

Playing as an archer, I had a conversation about bows and training at it with Sera. Pretty sure it never came up in my first playthrough as a warrior.

Yeah, that was cool when you get to talk to
I'm playing as an assassin for one of my characters and Dorian asked me about it
Leliana got into a discussion when my rogue specialized as an assassin and in a companion mission on another character an NPC I met noticed my specialization as well.
Choosing Reaver causes Iron Bull to get turned on by your new scent.

Which spec/companion mission? I'm curious.
Iron Bull tells interesting stuff about the Qunari (or rather, Qunari blackpowder.. or something) if you pick Tempest
Vivienne has something to say about Knight Enchanter, of course.
And then there's this:
Rogue
Knight Enchanter
Not sure if the last one is KE specific, or just a general mage thing.
The rogue though... I didn't even use dual blades, so I suppose it's rogue specific, not Tempest specific (definetly not Tempest, cause my armor isn't glowing with fire/ice/lightning)
I know for Knight-Enchanter I got a conversation with Solas and Vivenne about it, and mage in general got a few unique dialogue choices.
It's not a ton of dialogue, but it does seem to be more than the other Dragon Age games gave you.
As tempest I got convo with Bull and Sera.
As artificer with Varrik.
Cass, Cullen and Dorian mentioned about me being a Templar.
Sera and Solas talked to me about necromancy.
And finally Bull and Sera talked about me bing a champion.
Necromancer gets a remark form Josephine and, one form Solace that makes you question it, then Cole may, if you are not determined, make you start a new game and dump your Necro then and there. I finished that one but, I won't choose necro a second time - sorry about doing that, Cole.
It was supposed to help branch the story in a specific direction, but then they didn't have it do that and you get like a war table mission and 6 lines of dialogue.
"You'll only have one, but playing a one will impact the story in ways that we've never had the resources to do before!"

Mages stand out for me. There's a difference in, say, why the Chantry denounces you or how the Lord Seeker calls you out.. or whatnot. Compared to my warrior. It's like they just hated my warrior for no reason. With the mage, it made more sense.
Specs...not so much or too much more than others.
Human nobles get quite a bit of unique dialogue as well.

Nope. my female quizzy became a reaver. Bull want to jump her bone even more after that.
