What? Where? How?
This is my second full play through, Achievements done (finally), but this LOCKED DOOR?
Does anyone know what to do?

Emprise du Lion, at the Tower of Bone.
What am I missing? Where is the access? How do I unlock it?

I think you mean the Warden cave, it shows as a locked door before you discover it by finding a key on a dead body. There's sort of a canyon near there that if you run along it there is a dead body and you'll get the "there's something hidden here" indicator. The thing is a key that opens the cave (which will then show as a cave on your map). There's also a landmark in there.

If you move the reticule over Suledin Keep, look directly East (and just slightly north), there is a locked door at a tower before you start down into the red lyrium mines.
It is south of the camp at the Tower of Bone (and just slightly west).
Second time through. It is still there.
Hrm, I'll try to take a look in the morning... If I boot up the game now I'll never make it to bed and it's already late 
I saw that too. Wandered around for 15 minutes trying to find the door, but never did.
It's not the Warden Cave.
It's a door marked east of Suledin Keep, at the spot of the tower next to the quarry entrance.

It's a bug. There's a tower on that spot that's supposed to be locked, but it isn't. Just like a lot of locked doors don't 'clear' from the map when you unlock them.

Just going to chime in and say I also think this is the case. In two plays now I've cleared the entire Emprise of everything but that one thing remains.
Here's a screenshot of the map location.. Had same issue, was beating my head against the wall trying to figure it out.

Yeah, it's a bugged door that doesn't clear even if you have the Deft Hands perk, it always throws me off each game because I keep thinking I'm missing something as well.
