Stealth assassins will eventually come out of stealth and attack you or your party members. There is no theory here, just facts. The bug that was introduced with the DLC happens because certain critters (or more rarely normal opponents) get stuck inside terrain, usually having been knocked there as a result of an ability (Firestorm is notorious for causing this, although not always).
Other times an opponent wont be killed in a way that the game is able to register the kill, aka its health will be reduced to zero but its model will stay up (with no target reticle mind you) and you can still shoot it and perform abilities on it. Sometimes destroying that model through an ability (mostly as a result of Necromancer's walking bomb) may cause the key to drop.
These have all been confirmed after countless games and have been reported in the DAMP forum. They certainly don't seem to be in a hurry to fix this.

He's talking about multiplayer. Maps are divided into five parts, each separated by locked doors. The key to the locked door drops from the last enemy you kill on a given part of the map, then you advance to the next part.
Except that last year's DLC update to MP introduced a bug where the key doesn't drop, leaving your MP group in a map part devoid of monsters, with a locked door they can't open and advance. They can kick the host to try again and hope the key drops this time, or quit. If they quit, they forfeit all XP.
Makes people pretty grumpy.

'Seem' simply means they are not talking about what they are doing, they are probably fixing it or, unlikely, not fixing it.
Debating the matter ahead of time rarely reassures and makes a rod for their back if they struggle or are delayed.
Any dev who chirps up will be a lightning rod for debate and questions they can't answer.
All that they could meaningfully say is to recognise that they know about the issue.
And we know they know (many devs do read these forums more than many think they do).
BioWare and many developers have previous form on this, the time you hear about the fix is when the fix arrives.
So thread 23 on the topic is not really adding anything, though I recognise the frustration I've felt myself sometimes.
The holidays certainly haven't helped.
We know more updates/dlc is inbound at some point.
Releasing that without a key drop fix would hardly be credible so I think it's coming 'soon' TM.

Believe me, I do understand the frustration - I get maybe 5-10% glitch, but when I get 4 in a row, that's annoying...

I do understand and I'm a pretty patient guy. The occasional glitch doesn't really bother me and I hope that they are working on it. It's when you have 4 games in a row that can't be finished it's a bit ridiculous. So whether it's thread 23 or 123, I stick by the post.
You are way to kind saying that its Occasional .. as it sure is not. this happens 6-7/10 Online matches atm...of the 11 last maps we've done, we have been able to complete 4... due to the key bug... they need to sort this out before pumping out content... silly Bioware!
Edit : thats on PS4 btw*

The other problem is when meteors are cast with elementalist on animals. Not always but often.
Typically this doesn't happen when the leader has a fast latency. If the game is lagging and you notice bad rubberbanding than expect the glitch. If you have a real good connection to the server it typically doesn't happen.
These are what I noticed

oh makes sense now. Although if he said completing the level versus level up I would have gotten it sooner

had this happen for the first time the other day... thought we had missed a mob, well we did, but it wasn't one that drops a key... honestly wasn't surprised it happened
