HI! I'm playing the game trying to get the achievement of not dying during the entire campaign. So far I have completed Redcliffe and the Broken Circle succesfully, along with some Denerim sidequests, without dying. The problem comes at the Brecilian Forest, when I get to a camp in which you are attacked by a spirit (your party "dies" and you are left with Morrigan alone to fight this spirit). Technically my Warden doesn't "die", it is a scripted sequence (after a conversation, your entire party, save for Morrigan, falls unconscious). Does this count as dying for the achievement? I have a previous save, but I'm not sure if I should continue this playthrough or I should simply reload and continue from there.

I believe it does, and it's not a scripted event. The reason your party feel unconscious is because their willpower stat isn't high enough.

The campsite with the Shade, right?

If so what happens seems to also depend on what choices you make.  It is possible to have everyone up and ready to fight the shade when it shows up, and as such to not get an injury without investing in Willpower.  You basically need to try and leave at the first opportunity (more or less, the mechanics have to do with a check that is generated through the dialogue, if you fail then the character with the highest Willpower is always awake).

when trying to get that achievement I made sure to ignore that campsite all together because I do believe that counts as a "death" as there is injuries to your character when you are revived.

Thanks for the replies! I ended up loading a previous save and then skipping the campsite. I realised my Warden had died because he was injured. I didn't know it was tied to willpower, I just thought that Morrigan, as a mage, was able to resist the shade. On my previous playthrough my mage also resisted the spell. It probably happened because the mages are usually the ones with most willpower. Anyway, I will probably avoid that place for the rest of my playthrough.

Yes, it does count as an injury as reflected in the character record screen, and as such won't unlock the achievement.

As have been mentioned above, try leaving the camp immediately after the initial dialogue sequence, that is, without examining the fire, bedrolls or the tent. At that point, another dialogue sequence should trigger in which the party will consider staying and resting for a little while; end the dialogue by choosing a 'suspicious' option. After that, attempt to move away from the camp once more; the shade should ambush the party at this point, thereby preventing the injury.
