I found this from a forum post, these are the level range of all zones according to the Prima Guide and it seems mostly correct:
Haven - L1-4
Hinterlands - L4-7, L8-11, L12+
Val Royeaux - L4-6
Fallow Mire - L8-10, L12-15
Storm Coast - L7-11, L13-15+
Forbidden Oasis - L8-12, L5-15, L18-20
Skyhold - L10-12
Crestwood - L10-12, L13+
The Western Approach - L10-13, L14+
The Exalted Plains - L10-13, L13-16
Emerald Graves - L14-16, L16-20
Emprise du Lion - L16-19, L19-22
The Hissing Wastes - L19-23
If Bioware removed the enemy level scaling CAP on most of the entry level enemies in these zones, a lot of the issues with difficulty could be resolved.
The level scaling i believe is a huge contributor to the drop off in difficulty (yes, Nightmare). I think it's why many people end up over-leveled quickly after Haven. The other being a lot of really random SMALL groups of enemies and sometimes large desolate areas that bring no challenge.
In Your Heart Shall Burn is the most difficult quest in the game that scales high, and then upon getting to skyhold we get all these low level scaled zones.
I don't think the scaling was really tested thoroughly, I found a lvl 6 purple bow (farlayer?) in exalted plains...how anyone would still be lvl 6 when getting to exalted plains is beyond me (In Your Heart Shall Burn is lvl 8-11 recommendation, so why do we even have any lvl 6 gear after HAVEN?).
Same with Cassandra's quest Promise of Destruction. It requires killing groups in all the mid-range zones first( exalted, western approach, emerald graves, etc) then you finally get the quest, you go to do it, the gear dropped is lvl 6-7 along with the boss being low level...a super disappointing reward to what was supposed to be an epic quest. There is also no challenge when this quest could of easily just been scaled up to be appropriate level, multiply the enemies and with scaled up rewards. Would of made a killer quest.
I think Bioware had the game and certain zones scaled differently at first, and then changed it all around while forgetting to scale up some items and enemies after adjusting everything.
