My world state imports seemed to be working perfectly, so after taking several trips through the character creator, I got lax about checking my playthrough states in the Keep. Once I finally settled on a character I liked, I got 9 hours into the game when Leliana told me that the Warden was dead! I went back to the Keep to check and sure enough, my playthrough state had a default Warden/Hawke in it. Now I'm can't be sure if that was a bug because I was so tired I may have accidentally clicked OK on the default world. But strangely, there was also no image for my Inquisitor on that playthrough state, even though all my previous attempts had them. Just a blank gray space where the image goes.
I made a new character and imported my custom world. When I checked that one, the Warden and Hawke seemed fine, but the Inquisitor was still missing. I was starting to get curious, so I exported a new world state and made yet another character. This time, not only was the inquisitor missing, but the space where she would go wasn't there either. It looks just like one of the editable world states with Hawke and Warden side by side.
Now as far know, the keep doesn't really support Inquisition choices yet, so maybe it will be fine. But maybe it's a symptom of a more serious bug. Should I be worried?
