ASOIAF/Game of Thrones: Total war.
Seriously. The awesome strategy of Total War combined with the world and politics of westeros would be awesome. Historical battles could be replaced with battles from the series (battle of blackwater, battle of the wall, etc, etc), the dozens of different factions would create a giant selection of factions, politics could take a larger role in it than previous total war games, we get to fight in Iconic locations (moat calin, Kings Landing, Castle black, etc), there could be an expansion pack for Essos and god-dammit creative assembly, just start talking with George RR Martin!

Also, I'd kill for a Dragonfall-type game in the Burn Notice universe.

A "survival RPG" set in Eastern Europe at the time of the Mongol invasions. The goal isn't so much to beat back the Horde but escape it with your family to the west. We need more historical RPGs in general.
A Lord of the Rings game set in Harad. Basically a combo between Shadow of Mordor and Assassin's Creed where you work to expel Sauron's influence from the lands. I'd like to see some of Tolkien's concepts explore more (Troll-men, Wraiths) along with added elements from African and Middle Eastern fantasy.
Also an actual Warhammer Fantasy/40k tabletop game. I'd like to see it represented with special effects.

My idea is a game with real, "alive" NPCs. NPCs who have a job and make money in the game world, who must buy food to eat in the game world, who would starve if lacking food in the game world, who have opinions of each other NPC they encounter in the game world.
Picture a game world with several farmers feeding a small town. If the player character kills one off, there's less food harvested. if there's less food harvested, there will (might) not be enough food. If there's not enough food, NPCs will do what it takes to get food: steal from the store, perhaps, or go out to the world and start hunting (or start a farm if they're farming-inclined--they'd have job affinities).
Hunting too, there would be a diverse ecosystem. If the player hunts deer exclusively, the deer population would die out. Creatures who eat deer, such as wolves, would have to go elsewhere for food...perhaps a different animal on the food chain. Perhaps the animal at the top of the food chain...
I want it to be, at its core, a survival game. But almost overshadowed by a strong systemic cause-and-effect system. The man who kills a farmer and thus begins a cycle wiping out the ecosystem and town. The man who hunts deer to extinction, causing a pack of wolves to hunt him down and wait outside of his hut for him to exit...and feast. The butterfly in the storm. The Complex Effect.
That's a terrible name, isn't it. Anyway, while this sounds crazily ambitious, ultimately i don't think it would be, too much. I think it would mainly take extensive, extensive scripting, to define all of these player, NPC, and animal attributes. To include all the different "jobs," different "actions." To incorporate some real diversity in how the NPCs approach things.
