Just putting this out there in case anybody who reads this has any ideas. I'm missing 4 codex entries, aside from those I can't get or are bugged obviously, and I'm inclined to believe they're placeholders for Black Emporium content that were added with Patch 6/Jaws of Hakkon. 

 

#42 in Characters, after Clarel (Xenon?)

#27/28 in Places (The Black Emporium? It's after The Anderfels, The City State of Kirkwall is in there somewhere as probably #28 and then it's The Cradle of Sulevin)

and then #19 in Maps

 

These numbers are all for the English version of the game, for reference. The only one that kinda throws me off is the addition in Maps, but I suppose BE could be adding one. I think I'm going to let this go until BE gets released, but if anybody with the update/DLC actually DOES have any of these entries by chance, the names of them would be appreciated.

The counts are all wrong now. It looks like the patch 6? added some of the DLC codices in the front of some the lists, and in the middle of others.

I can't believe Bioware repeated the mistake of Origins' codex entries in Inquisition. Why even have numbers in the first place if the get jumbled with every patch/DLC? They aren't useful in the least if they are different for everybody. Also why show the empty codex entries that you will never be able to acquire?

#24 is Blood-Splattered Notes which according to the list here was originally #20. It's in the Emerald Graves! #46 is Leather-Bound Hakkonite Journal, which is found near one of the Ocularum in the Frostback Basin.

Were the DLC codices compiled? I feel like some of mine are still missing even after cross-referencing here and on the wiki.

 

Also, a theory on the Kirkwall location codex, which I'll test my next time around...could it be that we get the Kirkwall codex if we speak with Hawke about HLTA before speaking to Josie about WEWH? I think the last time I spoke with her first and didn't get the codex, but when I went directly to Hawke after arriving at Skyhold I've received it. I've definitely had the codex in some plays and not others, and usually with the same Keep state.

The list is, unfortunately, still not updated for the various DLC. Numbers may have changed. I have no idea when I'll find the time and inclination to add the new entries.

As for the Kirkwall Codex entry, I had it in one PT as well, but haven't managed to get it again. I'll keep it in mind in any new PT.

I'm still looking for that one as well. According to the Wiki it's supposed to be in Skyhold's library. It's not in the OP (the summer edition would be the 5th part, there are only 4 in the OP). I've been through every nook and cranny there, and the small library downstairs, but I've never seen it.

 

Has anyone ever seen it?

I did some digging in the Mod Maker, and I think it might be bugged. Can't really confirm, though, thanks to Dunning-Kruger.

 

There are several other bugged entries beyond what's been identified as well, if the holes in my post-Trespasser codex are any indication. And it turns out I missed a couple in the main game anyway, so the completionist run's a bust anyway.

Okay playing DAI for PS4, and long past the point of talking to Hawke (i did all the options available when talking to them) and still didn't get The City-State of Kirkwall as a Codex entry. I'm fairly certain it's a bug. (I'm already in Trespasser so the point is moot but I figured I'd share that the issue still exists.)
