I'm a bit of the ways into the game and I can't seem to get the companion quests from Dorian, Solas, or Iron Bull. I can't remembering doing anything that they disapproved of, even slightly. Is there some other choice I made that's gating me out of their quests? I've seen people complaining about Dorian being bugged, but I don't know anything about Solas or Iron Bull.

Where are you in the game? Of course the first is recuiting them, the next ones happen after you do Wicked Hearts or, romance them. Keep progrssing, talk to them often, and if Dorian gets a letter about Felix, Go to the main hall IMMIDIATELY after that, do nothing else. You will miss the quest otherwise.

Dorian's quest is extremely bugged, so even with a high approval, there's a chance you can't get it or can't finish it.

Bull and Solas' quests both require higher approval to initiate their personal quests.

Easy approval for Bull is taking him to fight a Dragon or Venatori.

Solas' is harder, I had trouble initiating the quest when I conscripted the mages/allied with the templars. For allied Templars playthrough, I let Cole embrace himself as a spirit, which scored a huge approval gain. For the conscripting with mages, I had to be more careful, as there was a moment when Solas was particularly bitchy with my Inquisitor, but thankfully that Inquisitor was very open to the idea of spirits and mages, so after a few conversations, his approval went up again.

I'm after Wicked Hearts at this point in the game. I talk to them all often, but they no longer have anything new it seems. Dorian hasn't said anything about letters and it seems I've exhausted Iron Bull and Solas' dialog options.

It's totally possible that I don't have as high of an approval as I thought, but it just seems like I've never seen them disapprove of the Inquisitor's decisions.

I didn't get Dorian's quest either my last run, when before it fired without problems. We still stayed friends and he said he was going to stick around post game but some of the dialog was different. Solas' quests and his little dream trip he takes with you only seem to trigger if you constantly agree with him and ask the questions he likes. Express any doubt on spirit's rights vs. the danger of demons and he shuts right down. I try to avoid Bull's quest as long as possible as it just has no happy ending and the cutscene animations of the dreadnought and Venatori are among the worst I've ever seen in any big market game in the last 10 years. It's so jarring how bad that whole quest is.

I've had a two games where Solas' quest didn't trigger until almost the very end of the game (I think I just had the final battle to go when it finally triggered).  I've also had one playthrough where Blackwall's quest never triggered.  Both times my approval was just too low to make it happen.

Solas you can build up if you take him around and trigger the elven artifacts.

Iron Bull gives approval for drinking with him after slaying a dragon (that only happens after the first dragon you slay when you're at skyhold).  He also highly approves of killing dragons (with him in the party) and slightly approves of killing venatori.

I think you should do Dorian's quest early in the game. You should also talk a lot with him. Just ask him about everything possible in the dialogues. You know he loves talking. Especially about him self. It also helps to flirt a bit with him, even if you're a woman. The flirting can be a silent deal between you and him in that case. He likes it.

Iron Bull? You can wait a bit with his quest. Notice when he vanishes from his usual spot near Cassandra and resides in the inn. You find him just opposite the entrance door on a chair. Talk with him, and when he offers a drink just do it. The dialogue gives you the option to [Drink] three times. Do it!
Take the attitude as a war veteran soiled in booze and play tough (tough, but serious and honest though).

Solas I can't tell. I never had his personal quest. He never touches the subject. The only thing I've discovered is that he initially get pissed off and bables something about doing more for the elves. Wierd and out of logic.

What have your decisions been thus far? Who did you ally/conscript? What did you do with the Wardens? Who did you choose to rule Orlais?

Bull doesn't like allying with the mages or conscripting the mages. Solas doesn't like conscripting mages or allying with the templars. Bull approves of having the Wardens ally with the Inquisition, Solas doesn't. Supporting Celene as Empress is a pretty safe option in terms of approval, as is having Florianne arrested as opposed to doing her boss fight.

Dorian's personal quest must be done as soon as possible once you get the trigger. I had two main quests to do in one PT; Winter Palace and Adamant. I did Adamant and his quest never turned up.

I actually missed Solas's quest in the first PT; I think it's either I failed to talk much to him or I didn't quite get the approval needed. IB's quest is easy to kick start.

Yeah, the Dorian bug.... I'm glad I haven't encountered any of that nonsense.

Solas is a sourpuss and doing things that many players do, such as letting the Grey Wardens join the Inquisition will cause him to hate you. I didn't get Solas's personal quest until I was almost done with the game because he hated me throughout most of it.

Bull really wants to kill a dragon. That's the easiest way, but not the only way to gain lots of approval with him.

Solas i think usually his missions do require you to socialize with alot and you really need to open every topic he brings up and go with the "how so?" or "oh really?" or "you tried x?" type stuff. this seems to always improve your relationship with him easily. I know his early one requires you taking him to Hinderlands while a later one takes you to Exalted planes. Frankly I never have issues with Solas's missions.

Dorian I feel can be confusing because he has several needing to be done on the WAR TABLE (mostly on the Orliasian section) while others like "One less Venetori" require you taking him with you to kill venetori groups. I can honestly see why this can be problematic since he has several WAR TABLE and actual combat relationship quests.

Iron Bull is a tad bit the same in a way as Dorian and Solas because I think when you first get him you need to do a few WAR TABLE missions but most do require actual combat. For every time i've played I have noticed and as dumb as it sounds you have to not only socialize with Iron Bull but his right hand man (woman....whatever) Krem as well. Like Solas when talking to Krem and Iron Bull you need to open every social tree as much as you can and make sure everything is disgussed. Also when you talk to Krem you need to choose "How can I use your group?" social tree choice and she would give you WAR TABLE missions. These are a pain to find on the WAR TABLE but these are required to do as well. After doing enough of Iron Bull's stuff and Krem's stuff you'll get more scenes involved from the two and Iron Bull's group the charges. If Krem says "I can't think of anything else" when you complete the WAR TABLE missions don't fret, just keep hanging out with iron Bull.

I don't think it's just the Wardens joining the Inquisition factor. If you disagree with him regards the Fade, that won't score many points either. I always agree with his views on the Fade, recruited the Wardens and I still get his quest.

I didn't bring IB on dragon bashing or on other missions, I still get his quest. And yes, I also talked to Krem and get quests from her.

You need to have high approval with them...though Dorian is bugged(this needs to fixed and fast).
