I started buying medium chests a couple of weeks back, and regularly only receive 5 items, rather than the 6 described. What gives?

Yeah, it happens.  Items also mysteriously vanish from large chests as well.

I've seen multiple YouTube videos of people buying Large chests and only getting 9 items (4 real items, 5 potions).

It's probably your dial up connection holding on to the rest of them.  You should receive the lost items in 2-4 weeks.

Also I seem to be getting some issues with chests showing items as rares when they are in fact unique. This happened with the Starlight sword I got recently

The well appears to have dried up.

ive logged in multiple times to find that mysterious items have randomly appeared in my inventory.

For a disturbingly long time the best ice-specific staff I had was the acolyte version. I was dreading doing the "ice" challenge with that thing, but (1) apparently we won't have to do an ice-specific challenge and (2) I finally got a better ice staff.

As far as medium chests being better? I finally got the Sulivan blade from a...Large chest.

I don't think i have ever bought a medium chest to be honest.  always large ones, bought a potion pack just once and actually did buy two small chests last night, the reason?  After I had bought my two large chests I had 666 gold left over and I'm not walking into my next battle with the mark of the devil in my pocket.
