OP, I bet you don't have 100%. Or do you have every single Codex entry it is possible to get in one playthrough?
Otherwise, unfortunately, this is not a dynamic world as in the TES games, where you can go on doing things like randomly generated quests and feel like you're living in the fictional world. Your role as Inquisitor doesn't make that plausible either. So...replay with different choices or try to get those last elusive Codex entries.

i have a true 100% run
all collectibles and everything
no requisitions
no uncompleted quests
Well you must have a different version than the rest of us. For everyone else one of the hinterlands mosiacs is inside a rock on a cliff overlooking the wolfs den. Which no one has managed to get to it yet.

i found the last 'hinterlands' piece in the hissing wastes :I
i must have duped a piece
and maybe i do
rift mage and necromancer dont work for me
no weakness virultent triggers 25% of the time
i gain to bonus damage vs fear/weakened enemies
my game so broken
