One of my favorites was using Leaping Shot and flinging my poor elf off the top of the mercenary fortress in the Hinterlands. Amazingly enough it didn't kill her.

You might be talking about two different gates  - Andorral and Toth.  I'm pretty sure Andorral can only be opened from the other side, whereas if there is a lever to pull for Toth I am going to rip my hair out lol

The biggest one I can think of is definitely trying to kill Druffy when I was exploring the Hinterlands. I got him to very low HP at one point and kept on trying to chip away and realized like 20 minutes later that his health bar just didn't seem to be getting any lower so I looked it up online and yeah...lol 

 

Some of the agents gave me trouble like I missed the one in Orlais and at one point I made it all the way to the part where you kill Ishmael completely missing Michael de Chevin so I had backtrack to start the quest with him first.

 

Also not realizing earlier how useful hook and tackle (with the it beats walking upgrade) is!!

 

And this wasn't in this game but I did have two big D'oh moment in Origins where I was like 70% through the game and I was watching someone else play it and realized that 1. there were two weapon slots that you could switch back and forth from (wish they still had that tbh) and 2. you can pause the game mid-combat lmao i had no idea and i had been wondering why the game was getting so difficult because i had been playing all the battles straight through in real time -_-

I did that in my last game, but I didn't feel like backtracking and doing that fight over again.

Apparently right next to Cullen office there is a little trigger that gives you info on your troops and it changes over time.

What!    Where?  The amount of time I've spent hanging around Cullen's office you'd think I would have found this.   Clearly my attention was elsewhere.

 

You have to exit the office to the opening where the MP characters stand and walk toward the outer wall, there is an interaction trigger there. Took me 2 pt to notice. The information changes at least three times.

Wait what? So that gate can be opened without going to the other side? I thought that was supposed to be done that way only? How do you open it? lol

 

No that's exactly what I mean. :) I thought I would find the mechanism somewhere but in the end found myself opening it from the other side. When I looked at the map and realized where I stood I was miffed.

 

I thought 'the mechanism' was an obtainable item not a gear attached to the door. That's kind of a d'oh moment too I suppose.

 

Edit: As Freedheart said upthread though, there are two gates and the one I'm talking about is the one closest to the first camp in the zone. You eventually stumble upon it early on and never open it until you come from the other side killing giants and closing Darkspawn holes and whatnot.

I tried to get to Redcliffe too early in the story, took a shortcut and everybody was one shotted (bitten? Bitted?) by dragonlings. And then when I finally got there the guard wouldn't let me through the gate. Good times.

 It took me 2 and a bit 120-hour-plus playthroughs to see the gold direction finder on the search button, and that was only from reading about it here.   Man, I hated searching for those maker-blasted bottles.   Follow the noise, is it louder this way, or that way?    No noise, damn, go back to the start.    No wonder my pt's took so long.    Serves me right for ignoring the tutorial, I suppose.

Took me four playthroughs to even see the "move to valuables" button in the inventory... .-. Selling stuff is not such a pain anymore lol

Geez I don't even think I've noticed this area or the fact that they are the starting MP characters... and I've played some MP too.

Didn't realize you needed to unlock perks in order to get up to tier 3 focus.... and played through the game with tier 1 focus..... D'oh....

 

Also did not realize you can craft armor better than loot drops until half the game -- lost a lot of fights...

While in the Hinterlands I remembered there being some quest I was ignoring about killing animals for food - and then I saw a lone buffalo like creature and thought it would probably be an easy kill...

One of my stupidest moments was on my second playthrough, trying to find the last mage cache in the Hinterlands.

I found it with absolutely no problems on my first playthrough, but this time I spent ages running around the area, staring at the map marker and trying to climb a hill I knew was impossible to climb before I eventually gave up and looked online.

As it turns out, I'd spent about 15 minutes standing just a few feet away with my back to it whilst staring gormlessly up at the aforementioned un-climable hill.

 

 

Another which is more a massive flaw on my part - I recently restarted my alternative canon playthrough, losing about 30+ hours of progress because... I couldn't justify my Dalish elf having the Ghilan'nain vallaslin and wanted to give her Mythal's instead.

Which is still not as bad as replaying DAO (+DLC), DAA, DA2 (+DLC) all because I didn't like my Cousland's forename and wanted to change it (despite being aware of the DA Keep).

I think I know the one you mean. If we're talking about the same one, it's closest to the rift at Dwarfson Pass. I did the same thing in my current (3rd) game. D'oh.

Biggest d'oh moment: taking two full playthroughs to realize only in halfway through the third that materials with the "removes class restriction" description... remove class restrictions.

Gave Wintersbreath shield to Cass, not noticing it has Berserk on it, and she tanked for me for umpteen hours with it. On Nightmare. D'oh! Did a reasonable job, too -- now that she has a proper, non-Berserk shield, I assume she'll be indestrucible. At least I confirmed that KE is OP when I had to finish a couple dragon fights solo.

Lol I have one. First pt and get to Storm Coast for the first time. Walking along the beach and notice an awesome fight between a giant and a dragon! Start channeling Skyrim thinking I can maybe get my first dragon kill pretty easy if I let the giant tank it for me. So my party runs up to the giant to try to hit the dragon.

The dragon leaves after a few seconds... The giant didn't. :| Whole party smooshed by a boulder.

Oh and another d'oh moment probably caused by too much skyrim again: First play through constantly looking up at the sky when hearing and seeing a dragon fly up there and searching for almost an hour nearby for it, thinking it would land on me. Went half the game before I realized they had specific fighting areas. Lol

Oh it also took me my second play through to realize I could call a horse to ride. I assumed it was a background thing for the inquisition like the tents and stuff in requisitions when I went for Dennet. I completely missed the horse head on the wheel, even after opening it over a thousand times to use potions and party commands. That made me feel incredibly stupid. Lol

Crafting the best armor schemantic I had for warriors, pouring all my best materials in it only to then afterwards find out that it was for dwarves only and neither my elf-Inquisitor nor anyone else in the party would ever be able to wear it. :(
