I've had this happen also in that room. I couldn't do much cos I was an assassin, but the others were able to use basic staff attacks on it outside.
Interesting that enemies in DAI can leave the map without dying.
While in ME3 you'd have this happen:

Ahhh, so this is how despair demons are made. Take a fear demon, and isolate it in a cold environment.

Ive seen the DC jump off a cliff and kill himself in perilous before on elven ruins. Was quite funny.

I suspect the reason enemies in DAI don't (usually) die when outside the map is that they are not technically outside the map. The DAI maps consist of different sections, some of which are either open or blocked off based on the procedural generation.
I suspect the entire area is considered valid for an enemy to exist in, even though the PCs may not be able to reach that area due to blockages.

Enemies take fall damage. Fall Damage scales if I'm not mistaken. Done right enemies can deal massive damage to or even kill themselves with certain attacks at the edge of a vertical area like the top of stairs. Glorious things happens.

Yep, funny stuff happens with enemy fall damage
As for the DC killing himself, he tried to teleport onto a small platform outside the health font room boundaries. He went up, and then just kept going down the cliff.
