The specialisations for the Inquisitor can be opened up by completing a quest on the War Table.  I've never really kept track of when these open for each companion but perhaps it's at the same point at which the trainers become available to you.  That said, in my latest game, I have everyone's specialisation tree save for Vivienne.  I'm not sure why, but she doesn't have a specialisation tree opened.  Perhaps because I'm on bad terms with the b*tch.

Pretty sure you have to be level 10 as well.   Not entirely sure though, I usually don't get to skyhold before level 11.

Specializations open up shortly after opening up the lower area of Skyhold where the stables are (usually your first or second trip back there after getting to Skyhold). You may also have to be level 10, as the previous poster noted. As for the companions, their spec tree opens up the first time you take them out into the "real world" -- for companions I don't use much, I just go to a camp somewhere and change the party a couple of times until they're all covered.

Depends where you are in Main Quest.

The Specialisation is not actually linked to a specific level in this game. It only become available at a certain point in the plot. If you have not attempted Champions of the Just or In Hushed Whispers (you can only pick one of these two missions) you need to advance the plot.
