It's been awhile since I finished Origins, but I'm pretty sure both my Warden and Alistair survived.  Alistair became King, and my Warden made a pact with Morrigan and by giving her a child she saved my life.  Yet the dragon age keep does not let both survive...  whats going on?

None of the tapestry is pulled directly from any save game - you'll have to manually re-enter any flags, regardless of whether your game shows as synced in the Keep or not.  Syncing will only cause some other data, such as your custom heroes, to be shown in the Keep.

If you want to be accurate to the world(s) you've played before, you could fill in the choices you can remember (and which you probably also care about most as a result), or you could also just create a brand new world, choosing choices that seem the most interesting, even if you would normally have never chosen them while playing the game.  You could also re-install DAO and review the savegame data in that way, and then enter choices afterwards.

Neither approach is wrong - it just depends on the type of world you want to have.
