Fade-Touched Silverite and Obsidian (+X Guard/Hit) are easily best upgrades - great for everyone. Have 1 of each (FT Ob/ FT Sil) and you'll gain +8 Guard/Hit! But there are other good ones for weapons/armor....
10% chance to use 5 Hidden Blades on hit...
+7.5% extra damage for every enemy within 8 meters...
10% chance to grant Horn of Valor...
10% chance for Shield Bash on hit...
I find that warriors generate enough guard that Guard on Hit isn't as valuable for them. But then I only spec warriors as pure S&S tanks, so that may be a playstyle of mine.
I haven't found, to be perfectly honest, a masterwork I'm most inclined toward with a warrior on armor.

Yup, if you spec as S&S, Guard on Hit is generally redundant and you'll be much better off finding something else. That said, Guard on Hit works 100% fine for Two-Handed Warriors.
Guard on Cole is a lifesaver for him and allows me not to have to micromanage him so much.

Absolutely. I just haven't used 2H builds for anyone except a main Inquisitor for a bit, but in the end I've found that the 2H warrior is underwhelming even when it comes to its supposedly main role, which is burst damage. It's one throwback to DA:O and DA2 I kind of didn't except to see.
All my favs have already been mentioned, but I also like the "use 7.5 (or 10)% less stamina / mania" perks.

Fade-Touched Royale Sea Silk : Abilities cost 20% less mana/stamina if not damaged for 5 seconds.
When you're a rogue constantly running around in Stealth this one is awesome. Even against dragon if they're not using AOE attacks.
Another useful one (especially for your fast-striking squishies...):
Fade-Touched Snoufleur Skin: On Hit: Heal 1% of maximum health.
I like the fade-touched items that grant % chance of abilities. I find it endlessly entertaining. Right now Solas is running around with a suit that has 10% to use hidden blades. It's fantastic. Walking bomb and chain lightning are other favorites. Oh the number of times a stray fennec or nug was killed by Solas' chain lightning armor. Good times.
Putting "+guard on hit" on one dagger and "heal 1% max health on hit" on another dagger, makes a DW rogue pretty much unkillable and you never need potions.
+ 20%% focus gain is also a very good one to enable focus skill spam throughout your party.
For Black Wall I have some fade-touched item that makes him randomly gain Moving Fortress. It's easily the best upgrade for a tank imo.

All chain lightning masterworks are bugged all of them. They proc but all of them do 0 damage. Best defensive masterwork is heal 1% hp per hit and best offensive is hidden blades. If you want the best those are what you want.

I was contemplating having Caltrops for my two-handed warrior, do you guys think that would work?
Also I gave Dorian Essence of Perfection partly because it matches his character.
