Cullen is half-elf, that is the only explanation.
I have no better explanation for Cullen other than this one. 
That aside, I think Calpernia survived because Corypheus had to save her. Being his Vessel and all, he couldn't let her get killed.

Apart from Cullen's binocular vision, Samson apparently joins the battle for Haven at some point. His Codex entry is a warning issued by Cullen to his troops after the battle, in which Cullen references Samson's apparent prowess during the fight. If he personally engaged Inquisition forces, then Cullen has a very good reason for knowing that he was there.
He presumably survived the avalanche because reasons.

Yes the fact that everybody saw Samson and Calpernia across the whole valley is a bit farfetched.
Also many characters talk about how they saw Corypheus and you facing Corypheus even though he only appears when you are alone and everbody else should be far away to survive the second avalanche.

Yeah this bugs me too. If anything, I wish that the Herald/Inquisitor was asked some questions about it and you could choose to explain in detail.

I suppose that the people who saw this could have been the team you went to fight Corypheus with, since you send them running before the dragon shows up to destroy the catapult? Of course, what's weird about that is why didn't they stay to help you fight Corypheus, unless they were cut off from you and forced to peg it?
