It really is nearly impossible to find new or updated codex entries. The white-gold star used to mark new entries is more often than not lost against the backdrop of the codex card illustrations, which generally use a white-gold color palette. This would be easy for BioWare to fix if they changed the color of the "new" icon something that provided higher contrast, like bright green.
To illustrate the problem, here's two codex cards. Try to guess which is read and which is unread.

It's like the easiest thing to fix.
Why can't they just have a small patch include a fix for this issue regarding unread vs read codex entries being distinguishable?

As indicated by Dragon Age: Inquisition where the lore was richly expanded upon and as shown in countless threads on the forums where these developments have been and are still discussed.

Now, having acquired almost all Codex entries in various playthroughs, I have to say it's sort of a mess.
1. The entries are sorted alphabetically. Except for main quests. Except for one of them.
2. Whether an entry related to history actually appears in the history section appears to have been determined by rolling dice.
3. Whether a quest-related document you find in the world has a Codex entry or not appears to have been determined by rolling dice.
4. If you play "Champions of the Just", you can buy all the entries you missed from the other path in Val Royeaux, if you played "In Hushed Whispers", you can only buy some of them.
5. It's hard to find entries you just acquired in the Codex because they're marked as read. Only, not all of them because you don't always get to read them on acquiring them.
6. If you meet important characters for the first time, you get Codex entries for them. Only not always.
7. If you acquire a Codex entry during a conversation, you sometimes get to read it immediately, sometimes not.
8. Sometimes an important character has a Codex entry, but you don't get it at all in a complete playthrough even though that character makes an appearance in the main storyline.
9. Sometimes, an entry is marked as unread after an update, sometimes not.
I take it that the Codex is a low-priority feature, but I find the amount of chaos in it disconcerting. Aren't there rules about how the Codex entries should be handled that those who implement them are expected to follow? If this is a typical result of how game development works, I'd say there is room for improvement.
Can't say I've really been a fan of any of the codex in any of the Bioware games. Thank the Lord for the interwebthingeemabobwiki's.

Regrettably, it's starting to feel that way, as much as I'd wish otherwise.
But getting back to the subject, the Codex is pretty much a wholesale mess, both in the awful UI and the lore that frequently only serves to obfuscate rather than enlighten.

What's even better is that in one of the DLC's they decided to renumber everything. Someone on here painstaking cataloged all of the codex entries... and then some genius decided it would be a good idea to renumber them in a DLC.

Yeah, that was me - and it would've been completely ok if they renumbered it in a DLC if they also fixed the other inconsistencies, but that didn't happen.

They wrote a lot for the codex in DAI and hey they added pretty pictures!
But yep it's a mess , I still remember my first playthrough when I did the Snow Wyvern Heart quest with Vivienne, I go speak with her and click on a codex entry by accident , and quickly close it.
I speak with Vivienne , and she's kinda shady about her quest , so I decide to check the codex entry to clarify things.
I couldn't find it....

I agree with the majority.
I like the lore; it's good, well-written, and it really helps illustrate that there is a world out there beyond your character and your Inquisition.
But it's all in a lump.
Some of the quests require information hidden in the lore. No problem, some good-old-fashioned problem-solving via clues and notes is always welcome, but it would be nice if there were a way to "fast-travel" to the relevant codex entry.
If it's part of a quest that hasn't been finished, it's outlined in green (Inquisition likes green)
If it's part of a quest that has already been completed, it's outlined in grey (like the Wardens!)
If it's part of a quest you have ACTIVE, then it's outlined in gold (like Orlais!)
If it's "background" information, divide it by region (Ferelden, Orlais, Tevinter, etc.)
Then by timeline (Current, DAII, DAO, prior, Ancient, etc.)
If race is a relevant factor, then sub-divide by that (Human, Elf, Dwarf, Qunari, etc.)
Have the Fade be it's own category.
So on and so forth...
In summary, it's a veritable gold mine, full of feels, lolz, and genuine thought-provoking questions and observations, it just needs a little more accessibility.

Some quests actually have a function "Jump to relevant Codex entry". Unfortunately, like everything else in the Codex, this wasn't implemented consistently, nor could it be in cases where several entries would be relevant. Your color-coding scheme would work better. I doubt the feasibility of finer categories though. Some things aren't categorized that easily. On the other hand, some of the entries are really hard to put into their context if you forgot where you got them. There's room for improvement here.
