Yup, hopefully the next game will have a more organized codex. It's especially annoying when you don't read immediately the documents you picked up. No, I don't like to pause in the middle of a quest in order to do that, it not only disperses my attention but also is immersion breaking.
I like to read lore and stuff when I'm just chilling in safe havens.
