I have no regrets. I have the game guide, so I went through and charted every listed decision. I have a complete chart of my characters and decisions.
That I change almost daily.

I have a minor regret: I didn't stop Sera slaughtering that guy just to gain some approval with her.
I also regret not punching Bianca, or judging her and afterwards chopping her head off (but that was not an option to begin with).

Otherwise i"m content with Free Mages (Even though I'm Pro-Templar, I couldn't just leave Redcliffe messed up), Cassandra Divine, Warden Support, and Celene as an Inquisition Puppet.

I'm not sure if it was part of Cole's story or if I had a low approval rating, but the last time I interacted with him, he was fretting about whether he could have saved the real Cole. I made him a spirit, not human, and none of the options I said afterward changed a result. He disapproved of several big things, like giving Briala the boot, drinking from the well, going necro-mage, and after that last interaction with him, he no longer approves or disapproves of anything. Rescue slaves to red lyrium? Quiet. Save people? Quiet....
His dialog remained the same, but it was sad. He said "Forget," and I asked "Cole?", like, "Are you alright buddy?" And he said "You can keep calling me that if you want."
It felt like his spirit just died and gave up any desire. Maybe it was because he was a spirit by then and ulterior motivations were just gone, other than his desire to help. I felt more like I didn't care enough about his approval rating and it may have been avoided, but maybe it just wasn't. Eitherhoot, I would make Cole human, even if it pisses Solas off.
Letting the Chargers die.
I allied the Wardens the first time through. Shunned them the second time, and somehow I felt better about that decision. Whenever they decided to go full-out-idiot, it was justified.

At least Blackwall had the courage to try and be something better than what he was. How many assholes in Dragon Age's story ever realize they're shitheads and try to make up for it? Yea what he did was bad, real bad, but that's why he joined the Inquisition, and before then that's why he was helping people in the Hinterlands against bandits.
People really need to get off this guy's nuts.

Regarding that...
Anyway, I personally missed rescuing Flissa from Haven in my first playthrough, and I felt bad about it once I realized. I actually thought about it, thinking what persons were there, and didn't see her. I hoped she just got away, but... sure enough, second play I did find her.
Otherwise I think I acted like the character would in most cases I can think of, and made the choices I wanted... hardest decision on a meta level was definitely Hawke/Stroud. Absolutely loved my sarcastic femHawke and Stroud was actually cool I thought. I knew I would pick him since he just had less time to latch on, but really didn't want to pick anyone -.-

Gaspard makes Orlais more independent? Why would I not want them more dependent on the Inquisition? It would seem that the long term goal is to be the supreme ruler of the region, right?

My only regret would probably be the outcome of the Winter Palace (havn't finished the game yet, but i'm close). I wanted Gaspard to rule alone, ended up with Celene. All because I didn't get enough evidence. But hey, how was I supposed to know the idiot developers didn't put in enough Halla statues...

Stop with the Cole thing honestly. I've done both and I realized Weekes was right:
According to Patrick Weekes, there is no "win" option in this quest, as either choice has positive and negative aspects. As a spirit, Cole is happier and more at peace, and yet losing parts of himself; if more human, he is less happy but far more relatable. This was done to demonstrate that there is no right or wrong decision
